KNOW THE RISK
- Hazardous energy exists in many forms: electrical, mechanical, hydraulic, pneumatic, chemical, and thermal. Any of these can be deadly when released unexpectedly. [1]
- During servicing or maintenance of machines and equipment, uncontrolled energy can cause amputations, crush injuries, burns, or fatalities. [1]
- Lockout Tagout (LOTO) is the method used to prevent the accidental release of energy during these tasks. It involves physically locking switches or valves and tagging them to prevent reactivation.
- Without LOTO, someone might power up a machine while it's being repaired, creating a deadly scenario.
MITIGATE THE RISK
1. Implement a Written LOTO Program
- Ensure procedures are documented and easy to follow. Identify all energy sources for each machine and how to safely isolate them. [2]
2. Identify Locks and Tags by Person
- Each employee must use a personally labeled lock and tag. This prevents confusion and unauthorized removal. [2]
3. Only the Person Who Locked Out May Remove the Lock
- This is a core rule of LOTO safety. Never remove someone else’s lock, even if the task seems complete. [2]
4. Train and Audit Regularly
- Train employees on LOTO procedures and conduct annual audits to ensure ongoing compliance and understanding. [2]
